As the alumni association chapter officer, I hereby affirm my acknowledgement of all Fraternity policies, including the Risk Management and Membership policies, and recognize that noncompliance of these polices has no place in Alpha Phi Omega.
All alumni associations must submit IRS proof of completion by providing the Tax ID number and the annual filing acceptance receipt from the IRS for official recognition. Has the alumni association submitted the IRS SS-4 application for an EIN?
