Submit Alumni List Request

This form is for Alpha Phi Omega members, chapters, and alumni associations to request a list of Alpha Phi Omega alumni in a given chapter or geographical area. Alumni lists are provided as a service to our members, chapters, and alumni associations only, Furthermore, this information is to be used only for Fraternity purposes. They are not to be used or sold for personal or professional marketing. Anyone found violating this policy will forfeit all rights to future alumni lists.

Chapters, Recognized Alumni Associations, Extension Sponsors/Staff Members, and startup Alumni Associations are allowed one free list per academic year. Each additional copy is $5.00.

Note 1: If possible, please try to provide us with your national membership number (click here for an example) so that we can verify your membership status. We do not share this information with non-members (such as professional marketers and spammers). If you do not know your national membership number, leave the field blank and our system will attempt to perform an automatic lookup.
